Many patients require some form of continuing care after discharge from the hospital. The Case Management Department assists you and your family to begin to plan early on in your hospital stay for the day you are ready for discharge.

Case Managers coordinate with the rest of the health care team as well as with you and your family to develop a discharge plan to meet your individual needs. Some people require either home care services or a stay in a nursing home after hospitalization.
